When the San Francisco 49ers offense was on the sidelines, Alex Smith was wearing this cap. The offered cap was worn by the signal caller on October 4th, 2009 versus the St. Louis Rams for breast cancer awareness.

The flex fit cap (size "XL-XXL") shows light use, clearly acceptable given the fact it was used for three games.

Smith signed the front in black felt tip. The penning has mussed slightly. "11" is written on the top button. A PSA/DNA NFL Auctions certificate #N61805 will accompany the cap as authentication.
